Instructions

Parents/Guardians must carefully and accurately keep track of time spent working on any school work throughout the week.
Time must be logged under each day in minutes.
In addition to time spent on each course, Parents/Guardians and Students must carefully and accurately keep track of time spent working on Instructional tasks/projects outside of your Units or Online work (example: Reading, Science Projects, etc.)
For "Parent Signature", the parent or guardian must sign their legal signature. Please do not print or use initials. *Only students 18 and over may sign for themselves*

Parents/Guardians: Adjust attendance minutes according to the amount of time the student worked. Minimum Recommended Time per Day = 360 Minutes/5 Days per Week.

Select the week of attendance: Weekly attendance logs shall not be submitted for current or future weeks. Parents/Guardians can only submit attendance for previous week and prior.
